desired behaviors among the elderly in collaboration with the Provincial
Public Health Office and the Office of Social Development and Human Security 2. To develop a mechanism for
innovation expansion in collaboration with the Regional Health Committee for
People in Region 8 and the Thai Health Promotion Foundation (ThaiHealth) 3. To develop a model center for
preparation to enter elderly age through participatory action research 4. To develop a diabetes
prevention model for people preparing to enter elderly age by adjusting the
urban primary health care system |

4.1 Output |
1. There are 15 projects receiving funding support for
elderly health promotion strategic initiatives, covering 6 districts, with
366 project management committee members, 569 volunteer participants in the
area, 52 communities, 2,944 elderly participants, and increased health
behavior changes after joining the project. 2. A mechanism was established
for developing a diabetes prevention model for people preparing to enter
elderly age, with innovation expansion covering the entire health region of 7
provinces, resulting in 17 MOUs with organizations to implement the innovation. 3. One aging society preparation
center was established in the community, serving as a provincial model. 4. There were 80 participants in
diabetes prevention for people preparing to enter elderly age through
adjustments to the urban primary health care system, with 80% successfully
controlling their diet and losing weight. |
